Creators use the face paint slot for far more than paint, because it sits on top of everything else on the face. That makes it a handy place for freckles, birthmarks, glitter and tattoos you want without replacing your Sim's makeup.
The catch is that there's one face paint slot, so two face paint items can't be worn together. Want freckles and a face tattoo at once? Look for a version in skin details, which many creators include.
